Job Description:
The VNA Health Group is seeking a Full Time Hospice RN in Monmouth County.Responsibilities
- Participate in a two week clinical orientation
- Assess, evaluate, and develop care plans for patients
- Provide education and care strategies for patients and families
- Collaborate with VNA clinicians and external providers
- Document changes and update care plansQualifications
- Active RN License (BSN preferred)
- 1+ year of supervised RN experience (hospice, palliative care or home health preferred)
- Excellent communication and problem solving skills
- Car and valid driver's license
