Join a supportive culture where you will work alongside skilled
therapists, nurses, and caregivers. The Position In the role of
Occupational Therapist (OT), you will assess residents’ functional
capabilities and create tailored treatment plans aimed at enhancing
their independence in daily tasks. You will deliver hands-on
therapy, work with interdisciplinary teams, and ensure the
provision of high-quality rehabilitation services that align with
our residents’ objectives. Primary Responsibilities Assess
residents’ functional, cognitive, and psychosocial requirements
through standardized evaluations. Create and execute individualized
treatment plans to enhance daily living skills, mobility, and
safety. Deliver therapeutic interventions, including training on
adaptive equipment, therapeutic exercises, and functional
retraining. Provide education to residents, families, and staff on
methods to support rehabilitation goals. Accurately document
evaluations, progress notes, and discharge summaries in a timely
manner, adhering to regulatory and organizational standards.
Collaborate with physical therapists, speech-language pathologists,
nurses, and other team members to ensure coordinated care. Ensure
compliance with all relevant state and federal regulations.
Qualifications Valid licensure as an Occupational Therapist (OT) in
the state where you practice. Graduation from an accredited
Occupational Therapy program. Experience in long-term care, skilled
nursing, or rehabilitation is preferred. Excellent communication,
documentation, and organizational abilities. A compassionate
