Anesthesiologist for coverage to start as soon as credentialed. 3
month committment to start with possible extension. Please list any
anticipated procedures/surgeries the provider will be required to
perform and anticipated volumes: Pediatric Dental, General surgery,
ortho, Urology Description of Work Assignment: 7a-5p, guaranteed
hours per CAO, CMO request Breakdown of Clinic Days/Sessions and/or
OR Days: 4 days per week- 10 hour shifts Schedule of Days/Times
Needed to Work/ Shift Hours: 7a-5p, must stay in house even if no
cases until call team starts at 5p. On call? No. Patient Load: Less
than 150 cases a month Board Status Requirement (BC/BE) Board
Certified Is there additional support staff? 1 FTE, Joint ventures,
per diem, Monmouth Main staff Is the provider working solo or with
other providers? If so, whom? NA Are there Advanced Practice
Providers? If yes, is the locums provider responsible for them?
Physician model. There are no CRNA's. Attending physican solo
cases. Trauma Level of Work Location No trauma, no heads, no hearts
EMR System? EPIC Is the Clinic EMR Different from the Hospital EMR?
