Job Description:
Job Description Position Summary The Total Joint Nurse Navigator
serves as a consistent care coordinator ensuring timely access and
seamless coordination of services throughout the continuum of total
joint care. The nurse will provide medical information, education,
and share knowledge regarding internal and external support
services and resources. Will be responsible for facilitating rapid
scheduling, coordination follow-up care, and tracking patients to
ensure completion of recommended treatment plan. Will identify and
assist patient and families in eliminating potential barriers to
care by identifying and referring to appropriate resources. Will be
an integral part of and professional member of the
Interdisciplinary team promoting optimal patient care. Education
Current Registered Professional Nurse in New Jersey. BSN required,
master's degree in nursing preferred. Experience Minimum of 3-5
years of clinical nursing practice experience, primarily in area of
specialty. Skills National certification, Orthopedic Nurse
Certified (ONC), is preferred upon hire and required after one year
of employment if not certified upon hire. Job Location The Valley

Pay Range: $92,358.24 - $115,447.80 EEO Statement Valley Health
