GRANT FUNDED Job Summary : The SNUG Social Worker provides trauma-informed social work services for victims of violence and their families at Westchester Medical Center (WMC) and Mid Hudson Regional Hospital (MHRH). This role ensures seamless access to SNUG and OVS services, manages complex psychosocial and economic needs, and acts as a bridge between hospital and community resources. The social worker facilitates assessment, crisis intervention, discharge planning, and ongoing support, with the goal of reducing violence, improving patient outcomes, and fostering community safety. Responsibilities: Assess all victims of violence and facilitate transitions to appropriate community services, including SNUG and OVS. Provide psychosocial support and crisis intervention to victims and families, including immediate response during incidents. Complete initial assessments promptly after admission and develop discharge plans in collaboration with the interdisciplinary team. Serve as a resource for pediatric and adult victims at WMC (including Maria Fareri Pediatric Hospital) and MHRH. Coordinate with cover CM/SW for continuity of care and promote appropriate patient flow. Advocate for patient needs with community agencies, family, and friends. Collaborate with Risk Management, Patient Relations, Utilization Management, Case Management, and Quality Management to address potential problems. Facilitate physician partnership, interdisciplinary communication and rounds; escalate systemic or external barriers as needed. Maintain ongoing collaboration with community SNUG sites, responders, and social workers. Complete and implement SNUG outreach model training; comply with all required trainings. Provide ongoing support to victims and families post-discharge. Lead efforts to obtain charity and financial resources for victims. Participate in case presentations and complex care reviews. Educate hospital staff, leadership, patients, and families about psychosocial needs, discharge planning, and community resources. Supervise MSW students and contribute to the field. Maintain confidentiality of patient and hospital information. Practice in an environmentally safe and healthy manner. Assess for abuse or neglect and uphold mandated reporting duties. Participate in SNUG Educational Training (SET) and annual conferences as required. Document de-identified data for program evaluation and quality improvement. Principal Accountabilities: COORDINATE: Organize and lead violence intervention and support activities for victims and families. COLLABORATE: Establish partnerships with trauma teams, community SNUG sites, and external agencies. OUTREACH: Deliver crisis intervention, support, and follow-up for victims of violence. ADVOCATE: Secure resources and advocate for patient needs within the hospital and community. EDUCATE: Provide education to staff, patients, families, and leadership on psychosocial needs and available resources. REPORTING: Maintain accurate records, comply with OVS and DCJS requirements, and support program evaluation. Qualifications/Requirements: Masters Degree in Social Work from an accredited program required. 13 years of clinical social work experience, with a focus on care transitions required. LMSW Licensed Medical Social Worker (State Licensure upon hire) required. Knowledge of accreditation standards, healthcare regulations, performance improvement, and patient safety. Effective organizational, communication, problem-solving, program development, and computer skills. Ability to complete psychosocial assessments, risk factor analysis, treatment planning, and intervention models. Ability to build collegial and collaborative relationships internally and externally. Knowledge of health care law and regulations related to acute and post-acute care. Licenses / Certifications: LMSW Licensed Medical Social Worker (State Licensure upon hire) required. Other: Completion of SNUG Educational Training (SET) and ongoing professional development as required. Special Requirements: This position requires access to sensitive information, including HIPAA-protected data.
